When scheduling an appointment, a $50 non-refundable deposit is required that will go towards the $179 balance. This helps manage our schedule better, and don't worry, we Guarantee Approval or 100% Money Back. This service includes your required medical exam with our Certified and Licensed Marijuana Doctor, plus all refills and dosage adjustments for the state maximum allowed 7 months. Our highly experienced staff will also help first time patients with their state application. Again, a $50 non-refundable deposit will be applied to balance. Deposit is only forfeited with missed appointment without 24 hours' notice. One rescheduled is allowed. (State Application fee of $77.75 not included.)

If you are already a patient of Marijuana Care Clinic, follow-ups save $20.00 with this option by pre-paying with our $149 price!!! Payment must be completed 24 hours prior to appointment, otherwise the price changes to our pay-in-clinic price. Do you already have a medical marijuana card but want to switch doctors? Transfer patients must pay $50 non-refundable deposit to hold appointment and pay remaining balance in office. By securing your appointment with a deposit, we will happily give you a nice savings. Cancellation and/or no-show fee of $25 if cancelled within 24 hours of scheduled appointment or patient no-shows. (State Application fee not included.)
REQUIREMENTS
- Proof of Florida Residency
- Full-time, adult Florida Resident: Valid Florida state issued driver’s license or ID (with current address)
- Snowbird or Temporary Florida Residents: 2 separate proof’s of Florida Residency. Please, check our FAQ page for temporary Florida residents requirements.
- Minors (Ages 17 and under): Birth certificate or K-12 Registration with legal guardians name matching legal guardians Valid Florida state issued driver’s license or ID (with current address).
- Medical Records
- Medical records within 2 years of age from a treating physician, such as a primary doctor, therapist, psychologist, chiropractor, pain management, specialist, etc., including your QUALIFYING condition are REQUIRED.
- Don’t have a current treating doctor or recent medical records?
- If you have insurance, we recommend going through your insurance to get re-evaluated for a qualifying condition within your insurance group with a treating physician such as a primary doctor, therapist, psychologist, chiropractor, pain management, or any specialists.
- If you do not have insurance or want a rapid diagnosis, as a resource to our patients, we have a list of referring doctors below. You only have to choose one of the following and they will be at a seperate cost. As a courtesy, we will provide you with a 20% discount to your initial visit with us to help outweigh the cost of having to obtain medical records. Please, make sure you mention Marijuana Care Clinic referred you so they send us your medical records right after your encounter with them.

Documents to Bring to First Appointment
- Valid Florida State Issued Drivers License or Identification Card with CURRENT address
- Medical Records regarding your qualifying condition. Records MUST be within 2 years of age from each encounter date with Marijuana Care Clinic. The adequacy of records is determined per discretion of clinician. We DO NOT accept medications, medication lists, imaging (MRI's, CT scans, X-Ray's, etc), imaging reports, nor lab results.
- For Snowbird or Temporary Florida Residents: 2 separate proof's of Florida Residency. Please, check our FAQ page for temporary Florida residents requirements.
